Loan App Checklist

TIP – Derek will send you a personalized checklist once he knows your unique situation.

The following list of items have been compiled for your convenience and will give you a very good idea of what you will need to provide.

Please either bring these items with you for your initial consultation or email them to Derek in order to expedite the processing of your mortgage loan.

  • Your residence addresses for the past two years
  • Clear copy or original of borrower(s) driver license and social security card(s)
  • Names, addresses, phone numbers and contact person for all employers for the last two years
  • W-2 Forms for all employers for the last two years
  • All 1099’s for the last 2 years
  • Two years personal tax returns – 1040’s
  • Pay stubs covering the most recent 30 day period (all current jobs)
  • Two most recent months’ bank statements (all accounts, all pages)
  • Statement from retirement accounts such as 401(k), IRA, etc. (most recent quarterly statement, all pages)
  • Evidence of alimony or child support, a copy of the court order and ages of child(ren)
  • Copy of divorce decree and separation agreement if it applies
  • Social security and other benefit award letters if being used toward qualifying income
  • Documentation relating to any special issues such as bankruptcy, foreclosures or credit problems
  • Most recent mortgage statement(s) for loan(s) being paid off (refinance only)
  • Most recent tax bill and most recent homeowner’s insurance bill (refinance only)
  • Names, addresses and phone number of your landlords for the past two years (if applicable)

Additional Documents for Home Purchase Loans

  • Complete copy of the purchase contract signed by both buyers and sellers – may request from Realtor
  • Copy of earnest money check you sent to escrow
  • Your Realtor name and contact information
  • Name, address and phone number of condominium management company – HOA –  (if applicable)
  • Contact information for homeowners insurance agent
